Company Registration Information
Axon Medchem BV (Netherlands) is a limited liability company incorporated in Groningen, the Netherlands. It is officially listed in the Trade Register of the Chamber of Commerce (Groningen), as required for all Dutch legal entities. Registered customers may request a copy of this registration for trading purposes.
Axon Medchem LLC (USA) is incorporated in Virginia and registered with both Federal and Virginia tax authorities. It serves as the US and Canadian sales office for Axon Medchem BV. All US/Canada orders should be addressed and paid to Axon Medchem LLC. Contact us to request our Federal EIN or other registration documents.

Log in to your account and navigate to My Orders to view order history and status updates.
Updating Account Information
Log in to your account to update your personal details, including:
- Shipping and billing addresses
- Registrant name
- Newsletter subscription preferences
Request W-9 Form
US-based customers can request a W-9 Form. We will send the document via email or fax (please include your fax number) within 12 hours of receiving your request.
Returns & Replacements
